Is a Trapped Nerve Causing My Sciatica?

If you are suffering with sciatica, the nerve or nerve roots may not necessarily be trapped as such, but one thing is for certain, the sciatic nerve in some way, shape or form is being aggravated. The aim is to find out why it is being aggravated.

Firstly, what is sciatica? This term is used to impart pain which is felt in any place from the lower back, through the buttock and down the back of the leg to the tips of the toes. Not only may pain be experienced, but signs and symptoms such as pins & needles and numbness can be felt as well.

So what is happening then? Well, as I mentioned above, the nerve is not necessarily trapped. However, something is aggravating it sufficient to cause pain. This may be due to a estimate of reasons, for example:

1) A disc in the lower back may be protruding and placing pressure on the nerve roots which form the sciatic nerve.

2) Inflammation from the local facet joints of the lower back may be inflamed and this inflammation can aggravate the nerve roots which form the sciatic nerve.

3) Muscles, which the sciatic nerve passes whether through or adjacent to, may be placing increased pressure upon the nerve, especially if they are tight.

I suppose, with regards to the 1st and 3rd bullet points above, it could be argued that the sciatic nerve, or the nerve roots which form it, are trapped and therefore pain is resulting. However, that is a minuscule misleading, as it is unlikely that the nerve itself is trapped and cannot move.

Under these circumstances, the nerve will still be able to move, it is just that there will be increased pressure settled upon it as it does move, resulting in inflammation of the structures involved and subsequently pain.

In the case of the 2nd bullet point, there may be no pressure upon the nerve at all, it is just that localised inflammation is now affecting the nerve roots of the sciatic nerve and therefore pain results. I often use the analogy here of hitting your thumb with a hammer. Although it is likely you would only have hit a small part of your thumb with the hammer, initially most of your thumb will be painful, regardless of whether or not it was hit!

As with any pain, the aim of medicine is to address the cause of the problem. Therefore, if it is the disc which is the problem, the aim is to cut the disc back into its natural position. If it is inflammation from the local facet joints, the aggravating factor for the facet joints needs to be addressed, as if this is resolved, the inflammation will conclude and therefore no irritation of the sciatic nerve will take place. Finally, if it is tight muscles which are causing the sciatic nerve to be painful, the muscles responsible need to be identified and gradually stretched out.

The prominent thing to remember is that the cause of the qoute must be addressed. If only the signs and symptoms are treated, you may gain temporary relief but it is likely the pain will return again at a later date.

If however, you can avoid aggravating your thumb, the inflammation will conclude down and you will find you can do more and more with your thumb without aggravating the pain.

Unfortunately, low back pain or sciatica is a fairly common complaint while pregnancy, with in the middle of 50% and 80% of women suffering from this complaint. Once again, by spirited your lower back & pelvis in order to ‘Reverse the Curve’ the structures in the lower back which will be tightening due to the increased arch, will now be stretched out. This will further decrease the stresses being located across the sensitive structures in you lower back.

Exercise One: Posterior Pelvic Tilt

A good exercise for this is to lie on your back on the floor with your knees bent. The aim is then to flatten your back into the floor with a ‘back & downwards’ rotation of your pelvis.

If you find this difficult, you can try using your heels and lowest muscles to help out. As you exertion to flatten your lower back into the floor gradually tighten your buttock/gluteal muscles while very gradually pushing through your heels.

Exercise Two: Four Point Kneeling Stretch

Placing yourself on all fours, with your knees roughly under your hips and hands below your shoulders, gradually arch your back up towards the ceiling. Having done this, gradually lean backwards as if to sit on your heels. While doing this, you may feel a stretch in your lower back area, further up your back towards your shoulders or down towards your buttocks and even upper leg. As long as it is only a stretch you are feeling and not pain, this is no problem.

Nb None of the exercises given above should aggravate your pain. If they do, either ease off a tiny or temporarily stop doing them altogether until your pain settles a tiny more.

General Advice

As I alluded to above, the main problem with Low Back Pain and Sciatica while gravidity is typically the increased arch in the lower back. Therefore, other guidance which may help is as follows.

Abdominals: tiny and often while walking gradually tighten your stomach muscles for a few seconds (5-10) and then relax. The abdominal muscles are a vitally foremost muscle group when it comes to providing stability for the lower back. Therefore if you can tighten them tiny and often, it can help to prevent the transmit tilting of your pelvis.

Footwear: Not that I can dream many pregnant ladies walking colse to in high heels, but nevertheless, as a rule of thumb the smaller the heel the better.
